This document provides a detailed walkthrough for requesting a Faith's Law Employment History Review. Follow these steps to ensure a smooth submission of your employment history to your hiring entity.
To initiate the request for a Faith's Law Employment History Review, select the "Request EV" option. Provide the required hiring entity details and your personal information before clicking "Next."

Choose "Faith's Law Employment History Review (EHR)" along with any other necessary forms. Proceed by clicking "Next." At this stage, input all relevant information regarding your former employers.

If there are no applicable former employers, click the designated option. Begin typing the employer's name and select it from the dropdown menu. Then, click "Add Former Employer."

For adding more former employers, select "Yes." If an employer is not listed in the dropdown, opt for "Former Employer Not Found." You will be required to input the organization's name, email address, and phone number.

Click "Add Former Employer." Once all former employers have been added, select "No." Your list of former employers will be displayed below.

Click "Next" and thoroughly read the instructions provided for the applicant before responding to the self-disclosure questions. These responses will be submitted to your hiring entity.

Sign the document by typing your name. Then click "I Agree and Continue."

Review the summary page and click "Next." At this point, you may choose to upgrade for additional features or proceed with the basic service, completing the process.
