
This document provides a comprehensive overview of the process to add a care plan for a patient. It guides you through each step, from obtaining consent to submitting the care plan, ensuring that all necessary information is accurately recorded and saved.
Begin by opening the patient's profile where you intend to add a care plan.

Click on "Add Care Plan" and provide the necessary consent for proceeding.

Submit the consent and then click "Next" to move forward.

A pop-up will appear. Select "Yes" if you wish to bill for this time; otherwise, select "No".

Add all relevant diagnoses, chronic conditions, and allergies. Click "Save and Continue" to proceed to the medication section, where you can add or edit medications.

After clicking "Save and Continue", access the care questionnaire section. Here, enter your responses to the questions.

After answering the questions, scroll down and select "Save Changes" to ensure your responses are saved.

Click "Save Changes" to submit your responses in the care questionnaire. Without this step, your entries will not be recorded.

If you choose "Save and Continue" without saving changes, your entries will not be reflected. Ensure that "Save Changes" is selected first.

After saving changes, you can then click "Save and Continue" to proceed to the CCM information section.

Complete the important CCM information in this section.

Click "Save and Continue" to proceed.

You can now add the care plan by navigating to the appropriate section.

Ensure all changes and answers are reflected by clicking "Save Changes". Skipping this button will result in unsaved answers. You can edit and update answers if needed and return to the care plan section.

Verify the option to save changes is available and used.

Remember, saving changes in the care questionnaire section is mandatory for data retention.
