This document provides a streamlined guide for submitting the California AB2534 form. Follow the detailed steps below to ensure accurate and complete submission of your form, including adding personal and employment details, and authorizing the release of information.
Begin by selecting "Request EV" to initiate your California AB2534 form request. Enter your hiring entity, choose the correct name from the dropdown menu, and click "Save Hiring Entity." Proceed to fill in all required personal information and then click "Next."

Select "Forms" and choose the "Unprofessional Conduct Form" along with any other necessary forms like "Certified Experience," then click "Next." You will be prompted to enter all your former employers. Start by typing each employer's name and select it from the dropdown list.

Click "Add Former Employer." If you need to include more former employers, choose "Yes." If your former employer does not appear in the dropdown, select "Former Employer Not Found."

Input the employer's name, email address, and phone number, then click "Add Former Employer." Once all former employers have been added, select "No" to proceed. A summary of the listed employers will be displayed.

Click "Next" and review the authorization for the release of information. Sign in the provided box, type your name below, and click "I Agree Continue." A summary of your Experience Verification Request will be shown.

Click "Next" to proceed. Here, you have the option to upgrade to additional features or continue with the basic package. The process is now complete.
